ABSTRACT

RESUMO O propósito deste estudo foi reportar as alterações oculares observadas após picada de abelha com ferrão retido na córnea. Destacamos o tratamento e o desfecho de uma lesão de córnea incomum e sua patogênese. Trata-se de relato de caso e revisão da literatura de lesões oculares por picada de abelha. Paciente do sexo feminino, 63 anos, procurou atendimento oftalmológico de urgência devido à picada de abelha na córnea do olho direito há 6 dias. Queixava-se de embaçamento visual, dor e hiperemia ocular. Apresentou acuidade visual de vultos no olho afetado. Ao exame, notaram-se hiperemia moderada de conjuntiva bulbar, edema corneano com dobras de Descemet e presença do ferrão alojado na região temporal, no estroma profundo da córnea. A paciente foi internada para ser abordada no centro cirúrgico sob anestesia geral. Durante a cirurgia, o ferrão teve que ser retirado via câmara anterior, mediante a realização de uma paracentese e uma lavagem da câmara anterior, com dupla via e solução salina balanceada. Ainda não existe na literatura um tratamento padrão na abordagem de pacientes com lesões oculares por picada de abelha, sendo importantes a identificação e o reconhecimento precoce de possíveis complicações que ameacem a visão.


ABSTRACT The purpose of this study was to report the ocular changes observed after a bee sting with a stinger retained in the cornea. We show the treatment and outcome of an unusual corneal injury and its pathogenesis. This is a case report and literature review of ocular injuries caused by bee stings. A 63-year-old female patient sought emergency ophthalmic care because of a bee sting on the cornea of her right eye six days before. She complained of blurred vision, pain, and ocular hyperemia. She had glare sensitivity on visual acuity in the affected eye. Examination revealed moderate hyperemia of the bulbar conjunctiva, corneal edema with Descemet's folds and a stinger lodged in the temporal region, in the deep stroma of the cornea. The patient was admitted to the operating room under general anesthesia. During surgery, the stinger had to be removed via the anterior chamber, by performing a paracentesis and washing the anterior chamber with a double flushing and balanced saline solution. There is still no standard treatment in the literature for patients with eye injuries caused by bee stings, and early identification and recognition of possible sight-threatening complications is important.

ABSTRACT

Objetivo: Determinar el comportamiento de las urgencias oftalmológicas en los pacientes que arriban al Cuerpo de Guardia. Métodos: Se realizó un estudio observacional, descriptivo, de corte transversal, en 194 285 pacientes (103 859 en el año 2017 y 90 426 en el 2018). Resultados: El 52,3 por ciento perteneció al sexo masculino y 116 444 (59,9 por ciento) tenía entre 19 y 59 años de edad. La conjuntivitis fue el diagnóstico más frecuente, realizado en 72 574 (37,4 por ciento) pacientes, 43 186 (41,6 por ciento) en el año 2017 y 29 388 (32,5 por ciento) en el 2018, seguida de los cuerpos extraños superficiales en la córnea, con un total de 24 920 (12,8 por ciento), dividido en 12 544 (12,1 por ciento) en el año 2017 y 12 376 (13,7 por ciento) en el año 2018. En el 2017, 13 500 pacientes fueron diagnosticados de conjuntivitis hemorrágica como consecuencia de una epidemia que afectó a La Habana. Necesitaron ingreso 573 pacientes y de ellos 327 tuvieron cirugía mayor de urgencia, con una edad media de 45,11 años, y como diagnóstico más frecuente el trauma ocular a globo abierto con o sin cuerpo extraño intraocular. Conclusión: La conjuntivitis, los cuerpos extraños superficiales corneales y la hemorragia subconjuntival fueron los diagnósticos más frecuentes, y el trauma ocular a globo abierto con o sin cuerpo extraño intraocular fue la primera causa de ingreso y de cirugía mayor de urgencia(AU)


Objective: Determine the behavior of ophthalmological emergencies in patients attending the emergency service. Methods: A cross-sectional observational descriptive study was conducted of 194 285 patients (103 859 in the year 2017 and 90 426 in 2018). Results: Of the patients studied, 52.3 percent were male and 116 444 (59.9 percent) were 19-59 years old. The most common diagnosis was conjunctivitis, with 72 574 patients (37.4 percent), of whom 43 186 (41.6 percent) were diagnosed in the year 2017 and 29 388 (32.5 percent) in 2018, followed by superficial corneal foreign bodies with 24 920 (12.8 percent): 12 544 (12.1 percent) in the year 2017 and 12 376 (13.7 percent) in 2018. A total 13 500 patients were diagnosed with hemorrhagic conjunctivitis in the year 2017 due to an epidemic affecting Havana in that year. Of the patients studied, 573 required hospitalization, 327 of them undergoing major emergency surgery. Mean age was 45.11 years and the most common diagnosis was open globe ocular trauma with or without an intraocular foreign body. Conclusions: Conjunctivitis, superficial corneal foreign bodies and subconjunctival hemorrhage were the most common diagnoses, whereas open globe ocular trauma with or without an intraocular foreign body was the leading cause of admission and major emergency surgery(AU)

ABSTRACT

RESUMEN Se presentó el caso de un paciente masculino de 60 años, blanco, que fue atendido en la Consulta de Oftalmología, del Hospital "Dr. Mario Muñoz", del municipio de Colón, provincia de Matanzas. Refirió que llevaba aproximadamente 20 días, con sensación de cuerpo extraño en el ojo izquierdo, enrojecimiento, secreción constante, fotofobia y una lesión que le había aumentado de tamaño durante este período; la que se tornaba dolorosa durante el parpadeo. Se decidió realizar este trabajo con el objetivo de mostrar los beneficios obtenidos al concluir el tratamiento aplicado a este tipo de tumor (AU).


ABSTRACT The authors present the case of a white male patient, aged 60 years, who assisted the Consultation of Ophthalmology of the Hospital "Dr. Mario Muñoz", of the municipality of Colon. He referred feeling a foreign body in the left eye for already 20 days, presence of redness, constant secretion, photophobia and a lesion that had grown during that period, painful when blinking. The authors decided to write this article with the objective of showing the benefits obtained with the applied treatment in this kind of tumor (AU).

ABSTRACT

RESUMEN Se presenta el caso de una paciente femenina, de 15 años de edad, cuyo motivo de consulta fue la sensación de cuerpo extraño y aumento de volumen de coloración oscura en el globo ocular derecho. Antecedentes personales de trauma no referidos. Al examen físico se encontró una masa alargada, oscura, de bordes regulares y dura a la palpación, por debajo de la conjuntiva bulbar. Se realizó tratamiento quirúrgico y se extrajo un cuerpo extraño subconjuntival compatible con la punta de un lápiz, englobado en su extremo distal por un tejido fibrovascular, oscuro y con brillo metálico. Se exponen algunas consideraciones que pudieran ser la explicación para el comportamiento tisular adyacente al cuerpo extraño. Por lo poco común del caso se decidió su presentación.


ABSTRACT The case of a 15-year-old female patient is presented, whose main complaint was the sensation of a foreign body and increased volume of dark coloration in the right eyeball. Personal history of trauma not reported. On physical examination, an elongated, dark mass with regular edges and hard on palpation was found below the bulbar conjunctiva. Surgical treatment was performed and a subconjunctival foreign body compatible with the tip of a pencil was removed, encompassed at its distal end by a dark, fibrovascular tissue with metallic luster. Some considerations that could be the explanation for the tissue behavior adjacent to the foreign body are exposed. Due to its unusual nature, the presentation of the case was decided.

ABSTRACT

A Processionária (Thaumetopoea pityocampa Schiff), vulgarmente conhecida como "lagarta do pinheiro" é um inseto dos pinheiros e cedros, endêmico em meios rurais mas também em meios urbanos. A toxicidade ocular, rara nas últimas décadas pelo desenvolvimento de métodos de erradicação eficazes, é provocada pelos seus pelos e prevê-se mais frequente com o recrudescimento deste inseto. Revemos a epidemiologia da Processionária e as suas lesões oculares a partir de 3 casos clínicos. Caso 1: Doente de 64 anos recorre ao Serviço de Urgência (SU) com olho direito vermelho e sensação de corpo estranho após prática de jardinagem. A observação revela AVODc: 0,5, erosão epitelial, presença de um filamento no estroma corneano profundo, flare (++) e Tyndall (+++). Caso 2: Doente de 28 anos, recorre ao SU por dor intensa no olho direito acompanhada de hiperemia após contato com lagarta. Apresenta AVODc: 0,6 e Tyndall (+++) com presença de múltiplos filamentos (mais de 20) a diferentes profundidades da córnea. Caso 3: Doente de 26 anos, recorre ao SU por sensação de corpo estranho e lacrimejamento constante no olho direito, após realizar exercícios militares num parque urbano. Apresenta AVODc: 0,3, múltiplas erosões epiteliais puntiformes na metade nasal da córnea que recobriam filamentos de cor laranja e Tyndall (+). Foi instituída terapêutica com corticoide tó­pico e vigilância sintomática a cada um dos casos. A patologia ocular por Processionária decorre da toxicidade dos seus pelos, cuja migração ocorre preponderantemente no sentido intraocular. Inclui por isso lesões precoces (conjuntivite, queratite e uveíte) e tardias (catarata, pars planite, vitreíte e retinite). Os casos apresentados possuíam lesões iniciais, tendo recuperado totalmente do quadro inflamatório após seis meses mas mantendo os pelos inativos no estroma corneano. A gravidade destes casos prende-se à possibilidade de migração intraocular, que pode ocorrer anos após o episódio inicial, obrigando a uma vigilância ao longo da vida. CONCLUSÃO: O recrudescimento da Processionária, tanto em meios rurais como urbanos, justifica o conhecimento das lesões oculares que pode causar e o seu tratamento.


The Processionary (Thaumetopoea pityocampa Schiff), is an endemic insect of pine and cedar trees, existing both in rural and urban areas. Ocular toxicity, once rare due to the efficacy of eradication methods, is caused by the insects' hair and is expected to be more frequent with its recrudescence. We report two clinical cases and review the epidemiology of Processionary and its eye injuries. Case 1: a 64 year-old patient complained of red eye and foreign body sensation after being gardening. She had a BCVA of 0.5 and biomicroscopy revealed an epithelial erosion, flare (++), Tyndall (+++) and the presence of a setae in the deep corneal stroma. Case 2: a 28 year-old patient complained of severe pain in the right eye and hyperemia after having had contact with a caterpillar. He presented with a BCVA of 0.6, Tyndall (+++) and multiple filaments (over 20) at different depths in the cornea. Case 3: a 23 year-old patient refers to the ER with foreign body sensation and constant lacrimation in the right eye after having been completing military exercises in an urban park. He presented a BCVA of 0.3, multiple epithelial erosions in the nasal half of the cornea that cove­red multiple orange strands and an anterior chamber reaction (Tyndall +). Therapy was initiated with topical steroids and symptomatic surveillance. Ocular toxicity due to Processionary hairs, whose movement occurs preferentially towards the posterior pole, includes early signs (conjunctivitis, keratitis, and uveitis) and late signs (cataract, pars planitis, vitritis, and retinitis). The 3 cases presented had early lesions, having fully recovered from the inflammatory condition after 6 months. However, inactive setae were still visible in the corneal stroma. Intraocular migration, which can occur years after the initial episode, is possible and might be severe, thus requiring a life-long surveillance. CONCLUSION: Processionary recrudescence has been observed, both in rural and urban areas. It is therefore imperative to be familiar with eye injuries caused by the insect and its treatment.

ABSTRACT

We report a case of a retained graphite anterior chamber foreign body that was masquerading as stromal keratitis. A 28-year-old male visited with complaints of visual disturbance and hyperemia in his right eye for four weeks. On initial examination, he presented with a stromal edema involving the inferior half of the cornea, epithelial microcysts, and moderate chamber inflammation. Suspecting herpetic stromal keratitis, he was treated with anti-viral and anti-inflammatory agents. One month after the initial visit, anterior chamber inflammation was improved and his visual acuity recovered to 20/20, but subtle corneal edema still remained. On tapering the medication, after three months, a foreign body was incidentally identified in the inferior chamber angle and was surgically removed resulting in complete resolution of corneal edema. The removed foreign body was a fragment of graphite and he subsequently disclosed a trauma with mechanical pencil 12 years earlier. This case showed that the presence of an anterior chamber foreign body should always be considered in the differential diagnosis of idiopathic localized corneal edema.

ABSTRACT

A 37 year old male was referred to our centre for management of episcleritis with peripheral keratitis in the right eye. He had a history of ocular discomfort in the right eye of 1 week duration. Slitlamp examination revealed marginal keratitis between 12'o clock to 2'o clock positions in the right eye. Lid eversion revealed an insect wing on the tarsal conjunctiva along with an adjacent conjunctival granuloma. The area of the marginal keratitis corresponded to the area of the foreign body and the conjunctival granuloma. The probable mechanism of the development of marginal keratitis and the conjunctiva granuloma is speculated in this case report.

ABSTRACT

Orbital trauma usually affects the bony parts of the orbit; however, in rare cases foreign bodies are found within the orbit. In this report, we introduce a case with unusual large intraorbital foreign bodies (two parts of a brake lever) after a motorcycle accident. Although one of the foreign bodies was located in the posterior orbit, they required only one simple operation for retrieval. We will discus the management strategy.

ABSTRACT

Intralenticular foreign bodies comprise about 5 percent to 10 percent of all intraocular foreign bodies and can result in serious complications. The management depends on some factors like size, location, material type and the risk of infection. We present a patient with an intralenticular metal foreign body in the left eye that, following initial treatment with topical steroid and antibiotic, underwent lens aspiration with removal of the intralenticular foreign body and insertion of a posterior chamber intraocular lens with good visual outcome.


Os corpos estranhos intracristalinianos representam cerca de 5 a 10 por cento de todos os corpos estranhos intra-oculares. A conduta depende de alguns fatores, como: tamanho, localização, tipo de material e probabilidade de infecção. Relatamos um caso de um paciente com um corpo estranho metálico intracristaliniano no olho esquerdo que, após realizar tratamento inicial com esteróide e antibiótico tópicos, submeteu-se à aspiração do cristalino, retirada do corpo estranho e implante de lente intra-ocular com bom resultado visual.

ABSTRACT

A case of keratitis, due to an insect hair penetrating into the deep cornea, was reported. The patient felt right ocular pain while riding a motorcycle, as an insect struck into his eye. On examination, an insect hair was seen embedded into the corneal stroma with severe corneal edema, which caused a visual acuity of the right eye decreased to hand motion. The patient was treated by a topical antibiotics, cycloplegics, and anti-inflammatory drugs, without removing the hair. After a 6-week follow-up period, there was a spontaneous resorption of the hair. There was no apparent toxic sign during 6-months of follow up, and the visual acuity improved to 6/6. The insect hair could be left in the deep cornea with careful observation, and spontaneous resorption can occur.

A 65-yrs-old man referred for an ocular discomfort and red eye. The clinical diagnosis was leech in the left eye and its species confirmed as being Limnatis nilotica. Ocular leech infestation should be considered in patients with a history of swimming or washing their face in streams and lakes. Attention should also be given to ocular leech infestation in the differential diagnosis of ocular trauma with iris prolapse

ABSTRACT

An attempt is made to describe the ocular surface injury by foreign body among the people those who are working in welding, grinding, hammering factory. Corneal foreign body which is usually iron in nature is the most common problem among the people those who are working in these factories. Clinical presentation, clinical pictures, and management are discussed. Corneal foreign body is a common cause of ocular morbidity and loss of working hour in the work place. Most of the victims do not use protective glass during work. We conclude that protective glass will useful in reduction of these accidents and alertness of both ophthalmologists and their patients are necessary.

ABSTRACT

An unusual intraocular foreign body--the silicone sleeve of a soft-tipped extrusion cannula left behind accidentally in the vitreous cavity following a vitreoretinal surgical procedure for complex retinal detachment--is reported. The silicone sleeve remained within the eye for a year without causing any problem.

ABSTRACT

Introduçäo: A apresentaçäo clínica do corpo estranho intra-orbitário (CEIO) é variada. A conduta e o prognóstico väo depender da composiçäo do corpo estranho, localizaçäo e presença ou näo de infecçäo. Objetos metálicos e de vidro säo os mais frequentes e bem tolerados, ao passo que os orgânicos causam maior reaçäo inflamatória, podendo levar a sérias complicaçöes. É frequantemente difícil identificar e localizar o corpo estranhointra-orbitário apesar dos modernos métodos de exames de imagem. Objetivo: Avaliar a contribuiçäo dos exames de imagem no diagnóstico bem como a conduta adotada em pacientes com corpo estranho intra-orbitário orgânico. Métodos: Foram avaliados 3 pacientes com corpo estranho intra-orbitário de natureza orgânica após trauma penetrante. Resultados: todos os pacientes forma submetidos à remoçäo do corpo estrnho, apresentando melhora do quadro clínico. A tomografia computadorizada (TC) foi essencial na avaliaçäo, identificaçäo e localizaçäo do corpo estranho. Conclusäo: A identificaçäo pré-operatória, com auxílio da tomografia computadorizada, do corpo estrnho intra-orbitário foi útil para a conduçäo do caso. A remoçäo do corpo estranho de natureza orgânica deve ser feita com o objetivo de minimizar possíveis complicaçöes.

ABSTRACT

Three patients with intraocular foreign bodies and traumatic cataracts underwent single stage pars plana lensectomy with anterior capsule preservation, vitrectomy, removal of the foreign body, and intraocular lens implantation. The preserved anterior capsule permitted support for the placement of an intraocular lens in the posterior chamber in the ciliary sulcus. The procedure enabled early visual rehabilitation. This procedure seems useful in the management of posterior segment intraocular foreign body associated with cataract.

Se analizan 63 casos consecutivos de pacientes portadores de un cuerpo extraño intraocular de segmento posterior, que consultaron en el Servicio de Oftalmología del Hospital del Trabajador, en un período de 6 años. La vía de abordaje más frecuente fue la vitrectomía por par-plana y se obtuvo una visión final de 66,7 por ciento o igual a 20/40

ABSTRACT

Retained intraorbital organic foreign bodies, particularly wooden, are commonly encountered in ophthalmologic practice. We treated two children who had sustained such injury while playing. They presented to us with non-healing sinus with purulent discharge. In one of the patients, X-rays and CT scan helped to clinch the diagnosis, whereas in the other patient diagnosis was possible by correlating history with clinical findings. Surgical exploration in both patients helped us to remove the foreign bodies. Surprisingly, both the foreign bodies were 7 cm long wooden pieces. We, however, caution that management of such cases should be conservative and that surgical exploration be done only in case of complication. From our experience, we recommend proper localisation by all possible means, blunt dissection, careful haemostasis coupled with excellent lighting and exposure in the atraumatic removal of intraorbital foreign bodies.
